I understand that Save Our Youth “No More Sagging Pants” may modify, change, or revoke any of its volunteer policies, without my agreement. I hereby state that all answers on this application are true and understand that falsifying this information can lead to termination if accepted as a volunteer. I UNDERSTAND THAT IN ACCORDANCE WITH CITY POLICY, FINAL CANDIDATES ARE SUBJECT TO A STATE AND FEDERAL BACKGROUND CHECK, SEX OFFENDER CHECK AND DRUG TEST AS A CONDITION OF VOLUNTEERING. I hereby authorize any and all persons, companies, or agencies to release any and all background information, of a confidential or privileged nature, including criminal history, relevant to this application and any pertinent information they may have to the hiring authorities of Save Our Youth/”No More Sagging Pants”. I release all such parties from all liability of every kind as the result of furnishing the same to Save Our Youth “No More Sagging Pants”. This information is to be used to assist the SOY/NMSP Organization in determining my qualifications and fitness for the position I am seeking. Once we receive your volunteer application we will process it and run the required background checks. Once those steps are complete we will contact you to set up an interview. After the interview, if you are accepted as a volunteer with SOY/NMSP, you will be scheduled to attend an Orientation before you can begin your volunteer assignment.
